Ohio Tax Forms 2014 Printable State IT 1040 Tax Form and Instructions
Printable Ohio (OH) state income tax form IT 1040 or IT 1040EZ must be postmarked by April 15, 2015 in order to avoid penalties and late fees. Printable Ohio state tax forms for the current tax year will be based on income earned between January 1, 2014 through December 31, 2014. The Ohio state income tax rate for 2014 is progressive from a low of 0.537% to a high of 5.421%. This state tax rate can be found inside the IT 1040 instructions booklet.

Ohio State Income Tax Refund Status
To check the status of your IT 1040 income tax refund online, visit the Ohio Department of Taxation website and use their Check My Refund Status service. To retrieve the status of your OH income tax refund for 2014 you will need the following information:
- Social Security Number
- Date of Birth
- Type of Tax Return
The requested information must match what was submitted on your IT 1040 or IT 1040EZ state income tax forms for the current year. To look up prior year refunds, you will need to contact the Ohio Department of Taxation service center by phone, email, or mail.
