Printable Ohio (OH) state income tax Form IT 1040 or Form IT 1040EZ must be postmarked by April 18, 2017 in order to avoid penalties and late fees. Printable Ohio state tax forms for the current tax year will be based on income earned between January 1, 2016 through December 31, 2016. The Ohio income tax rate for 2016 is progressive from a low of 0.495% to a high of 4.997%. This state tax rate can be found inside the Form IT 1040 and Form IT 1040EZ instructions booklet.

The Ohio Department of Taxation will expect your 2016 income tax forms to be printed on high quality printers. Today's laser, bubble-jet, or ink-jet printers are generally acceptable. Ohio State Income Tax Refund Status

To check the status of your IT 1040 income tax refund online, visit the Ohio Department of Taxation website and use their Check My Refund Status service. To retrieve the status of your OH income tax refund for 2016 you will need the following information:

  • Social Security Number
  • Date of Birth
  • Type of Tax Return

The requested information must match what was submitted on your IT 1040 or IT 1040EZ state income tax forms for the current year. To look up prior year refunds, you will need to contact the Ohio Department of Taxation service center by phone, email, or mail.
